The Monroe County Health Department will be offering free COVID-19 bi-valent booster
vaccines at the following locations:
Wednesday November 16th from 11:00am-2:30pm at Oakdale Electric Cooperative (489
N Oakwood St., Tomah, WI 54660)
Wednesday November 16th from 4:00pm-6:00pm at Monroe County Fairgrounds-
Recreation Building (1625 Butts Ave., Tomah, WI 54660)
Thursday November 17th from 4:00pm-6:00pm at Cashton Community Hall (811 Main
St., Cashton, WI 54619)
Monday November 28th from 5:00pm-7:00pm at Norwalk Village Hall (208 Church St,
Norwalk, WI 54648)
Available bivalent vaccines include Pfizer for ages 12 years plus and Moderna for ages 6 years
plus. If interested in a COVID-19 vaccine for anyone ages 6 months plus, visit the Monroe
County Health Department office (315 W Oak St., Sparta, WI 54656) on Fridays from 10:00am-
6:00pm. The office will be closed on November 25th

The CDC recommends that everyone ages 5 years and older get a COVID-19 bivalent booster if
it has been at least 2 months since their last COVID-19 vaccine, whether that was their final
primary series dose, or an original (monovalent) booster. People who have gotten more than
one original (monovalent) booster are also recommended to get an updated (bivalent) booster.

vaccination card with you to your appointment. Anyone under the age of 18 years must be
