Appointment reminders will indicate if the appointment will be in person or over the phone. In-person appointments will be located at 315 West Oak Street in Sparta, WI. Participants can receive their nutrition education over the phone by calling the WIC office at (608) 269-8671 or complete on-line at WICHealth.org. WIC provides healthful foods, nutrition education, breastfeeding support, and referrals to health care and community resources.
